Klinnavartma – Quick look about the disease
Klinna = moistened, Vartma = eyelids
Klinnavartma is a condition of eyelids wherein eyelids appear wet and watery inwards.

Classifications

  • Classification based on the location – Klinnavartma is a Vartmagata Roga (disease occurring in the eyelids).
  • Classification based on Predominant Dosha – It is a Kaphaja Netra Roga i.e. it is caused by vitiated kapha dosha.
  • Classification based on Sadhya Asadhyata (prognosis) – Klinnavartma is a Sadhya roga (curable disease).
  • Classification based on predominant treatment / surgical process used in combating the disease – Klinnavartma is one of the eye disorders which have been included in the list of eyelid disorders wherein surgical methods are contraindicated (Ashastra krutya rogas).

Other names – Klinnavartma is also called by the names Praklinnavartma and Pariklinnavartma.

Klinnavartma, definition, meaning, pathogenesis
Klinnavartma is an eye disorder and occurs in the eyelids. It is named based on the wetness present in the lids. In this condition, the vitiated kapha causes painless swelling of the eyelids as visible from its outward appearance and lot of wetness and watery appearance inside the lids along with discharges. This condition is called klinnavartma i.e. damp / wet lids and is associated with itching sensation and pricking pain in the afflicted eyelid(s).

This condition is caused by vitiation of kapha dosha. It is a curable condition.

Praklinna varthma / Pilla vartma
The same condition explained in the name of klinnavartma by master Sushruta has been named as Praklinna vartma or Pillavartma by other authors. It is said to be presenting with extreme wetness of the eyelids associated with itching and mild pain in the eyelids. It is considered to be caused by simultaneous vitiation of all three doshas.

Praklinna Vartma – symptoms, correlation, treatment

Signs and Symptoms of Klinna varthma / Praklinna varthma eye disorder

  • Arujam Bahyatah Shunam – Painless swelling of outside of eyelids on the outside of the eyelids, as visible from outside
  • Klinnam – wetness / watery appearance on the inside of the eyelids
  • Sravati api – watery discharges from the eyes
  • Kandu – itching in the eyelids / eyes
  • Nistodam – pricking pain
  • Manda vedanam – mild pain

Modern correlation
Klinnav artma is a form of inflammatory oedema of the eyelids. It can also be compared to a condition called symblepharon. It is a partial or complete adhesion of the palpebral conjunctiva of the eyelid to the bulbar conjunctiva.

It might not be precise to compare these conditions because symblepharon is said to occur as a sequelae of trachoma or due to trauma. If trachoma is compared to pothaki, pothaki is not said to be a cause for klinna vartma and even trauma is not considered causal in Ayurveda. I have tried to compare these two conditions because the adhesion of palpebral and bulbar conjunctiva as explained in symblepharon looks due to excessive dampness / wetness or excessive secretions of the eye, which is a predominant symptom of klinnavartma.

Treatment of Klinnavartma eye disorder
Klinnavartma shall be treated on the lines of treatment of abhishyanda (conjunctivitis).

The measures include –

  • Tarpana – pooling of medicated ghee around the afflicted eye / eyes
  • Seka – stream pouring / sprinkling / showering of medicated liquids into the afflicted eye / eyes
  • Aschotana – instillation of eye drops
  • Anjana – application of collyriums
  • Nasya – nasal medication / errhines
  • Dhuma – medicated smoking etc

Klinnavartma is also called by the name Pillavartma or Pilla roga. The line of treatment of Pillavartma is also applicable for Klinnavartma eyelid disorder. The measures include –

Dosha harana – the morbid doshas of the head and body should be expelled and the body cleansed by the below mentioned measures –

  • Senhana – oleation
  • Siravyadhana – vein puncture / venesection
  • Shirovirechana – purgative errhines to cleanse the head
  • Asthapana vasti – enemas using medicated decoctions (read here)

Tarpana – pooling of eye with medicated ghee should be done after the doshas have been expelled.

Aklinna = not moistened, Vartma = eyelids
Aklinnavartma is a condition of eyelids wherein the eyelids stick to each other and there is absence of wetness and devoid of suppuration

  • Classification based on Predominant Dosha – Aklinnavartma is a Sannipataja Netra Roga (it is caused by simultaneous vitiation of all three doshas).
  • Classification based on Sadhya Asadhyata (prognosis) – It is a Sadhya roga (curable disease).
  • Classification based on predominant treatment process used in combating the disease – Aklinnavartma is one of the eye disorders which have been included in the list of eyelid disorders wherein surgical methods are contraindicated (Ashastra krutya rogas).

Aklinnavartma, definition, meaning, pathogenesis
Aklinnavartma is an eye disorder and occurs in the eyelids. It is named based on the absence of wetness in the lids. In this condition, the eyelids get adhered to each other in spite of washing and cleansing them regularly. They get repeatedly adhered to each other. The eyelids in this disorder do not go to the stage of suppuration. This condition is called aklinnavartma (sticky eyelids with absence of dampness and suppuration in the afflicted eyelid / eyelids).

This condition is caused by simultaneous vitiation of all three doshas. It is a curable condition.

Explanation of Aklinnavartma eye disorder by master Videha –
Master Videha explains aklinnavartma eyelid disorder as – ‘The eye disorder which in spite of repeated washing and cleansing the eyes remain stagnant with its lids adhered to each other is called apariklinnavartma. It is also called as pilla eyelid disorder’.

Signs and Symptoms of Aklinnavartma eye disorder

  • Dhautani dhautani sambadhyante vartmani punah punah – the eyelids, in spite of regular washing and cleansing would get adhered to each other repeatedly.

Modern correlation
Aklinnavartma, just like klinnavartma is a form of inflammatory oedema of the eyelids.

Some people have compared this condition to a congenital / genetic eye disorder named Blepharophimosis. In this condition the eyelids are closer to each other and looked sticking to each other as explained in Aklinnavartma.

But the comparable doesn’t look an apt one seeing that blepharophimosis is related to the development anomaly of eyelids at birth and we do not have a reference of aklinnavartma being a congenital condition. Also blepharophimosis, which means narrowing of eye opening is a part of syndrome known as BPES 1 wherein B=blepharophimosis, P=ptosis (droopy eyelids) E=epicanthus inversus (upward fold of the skin of the lower eyelid near the inner corner of the eye).

Treatment of Aklinnavartma eye disorder

The line of treatment of aklinnavartma eye disorder is said to be similar to that of klinnavartma eyelid disorder.

Aklinnavartma is also called by the name Pillavartma or Pilla roga. The line of treatment of Pillavartma as eplained in klinnavarthma is also applicable for Aklinnavartma eyelid disorder.
