Classification
Based on the location
Kumbhika is a Vartmagata Roga (disease occurring in the eyelids).

Based on Predominant Dosha
It is a Tridoshaja Netra Roga (caused by simultaneous vitiation of all three doshas).

Based on Sadhya Asadhyata (prognosis)
It is a Sadhya roga (curable disease)

Based on predominant treatment / surgical process used in combating the disease
Kumbhika eye disorder is a Lekhana Sadhya / Lekhya Netra Roga (curable by scraping).

Kumbhika, definition, meaning, pathogenesis

Kumbhika means the seed of a fruit resembling that of a pomegranate fruit. Since the swelling resembles the seed of a pomegranate or pomegranate like looking fruit, it is called Kumbhika.

Since there is a swelling or boil in the eyelid in this condition, it is also called as Kumbhika Pidika. Pidika means a swelling or boil or a pimple like structure.

Thus,
“Kumbhika or Kumbhika Pidika is a boil or swelling resembling the seed of a pomegranate like fruit, occurs on the eyelid and is caused due to the simultaneous vitiation of all the three doshas.

When swelling gets open and discharges its contents, it refills once again and gets swollen up. This keeps happening. This disease is curable condition”

Signs and Symptoms of Kumbhika eye disorder

Kumbhika beeja pratima pidika – Boil / elevated swelling resembling the seed of kumbhika, a pomegranate like looking fruit.

Features of the swelling –

  • Kumbhika beeja sama – resembles seed of pomegranate or pomegranate like looking fruit
  • Adhmapayanti bhinnayah – the boil / swelling keeps filling and swelling up even after the boil opens and discharges its contents

Modern correlation
Kumbhika can be correlated with internal stye / internal hordeolum.

Stye / hordeolum is an infection of one of the oil producing glands present inside of the eyelid, usually caused by staphylococcus bacteria and manifests in the form of an abscess / boil / lump or boil.

It is of two types –
internal hordeolum
external hordeolum

Kumbhika is often compared to the internal type of hordeolum. It presents with swollen lump and swelling of eyelid, mild pain and redness with burning sensation and light sensitivity, discharges from the eyes and crusting of eyelid margins.

Internal stye tends to come to a point on the underside of the eyelid. It presents with general redness on the outer and visible part of the lid.

Stye usually heals in 10-14 days. They are usually harmless and complications are rare. But they often recur. This point has been emphasized by master Sushruta in the description of kumbhika Pidika. Due to these reasons, Kumbhika Pidika is compared to internal stye.

Kumbhika is a lekhana sadhya vyadhi. It is one of the eye diseases which can be cured by scraping therapy.

Anjananamika

Anjananamika is one of the diseases of the eyelids (netra vartma gata roga).

Based on the location – Anjananamika is a Vartmagata Roga (disease occurring in the eyelids).
Based on Predominant Dosha – It is a Raktaja Netra Roga (caused by vitiated blood).
Based on Sadhya Asadhyata (prognosis) – It is a Sadhya roga (curable disease)
Based on predominant treatment / surgical process used in combating the disease – It is a Bhedana Sadhya / Bhedya Netra Roga (curable by administration of incision).

Anjananamika, definition, meaning, pathogenesis

Anjananamika is an eyelid disorder. It occurs in the form of a boil or swelling which has the colour of copper. It presents with burning sensation and pricking pain. It is soft in texture and on touch and is associated with mild pain. This condition is caused by vitiated blood. Anjananamika is said to be curable in nature.

Signs and Symptoms of Anjananamika eye disorder

Treatment of Anjananamika It is one of the eye diseases which can be cured by making incision (bhedana).

1. Swayam bhinna anjananamika chikitsa – treatment of anjananamika which breaks open spontaneously

Swedana – Aanjananamika should be subjected to sudation through hot compresses. After giving heat to the swelling when the swelling / boil / lump breaks open spontaneously, the physician should press it gently and remove its contents including pus.

Manashiladi Pratisarana – After giving sudation and pressing out its contents, the physician should gently apply / anoint the paste / scrub with the paste of the below said over the exposed part of the boil / edge of the eyelid –

2. Abhinna anjananamika chikitsa – treatment of anjananamika which doesn’t breaks open spontaneously

Swedana – hot compresses / sudation should be given to the swelling / boil.

Bhedana – boil / swelling should now be subjected to incision using a sharp sterile instrument. After incising the lesion, the pus and other contents need to be emptied from it.

Pratisarana – after evacuating the boil of its contents following sudation and incision, the exposed wound should be scrubbed gently or applied with the paste of rasanjana i.e. extract of Berberis aristata and madhu i.e. honey.

Kajjala Anjana – After scrubbing the wound, it should be applied with hot soot formed by a lamp in the form of collyrium.

All the above said procedures shall be done by an expert surgeon.

Anjananamika treatment as explained in Yogaratnakara –

Swedana – the physician should rub his index finger on his palm until it gets warm, and with that warm finger should give heat to the boil / swelling. It is in line with the rule that excessive heat or fomentation should not be given to the eyes.

Jalauka avacharana – since anjananamika is a disorder of the eyelids which is caused by vitiated blood, bloodletting should be a good option when treatment is concerned. Bloodletting from the swelling should be done by administering leech to anjananamika.

Anjana – silver should be rubbed over the palm and should be used to apply collyrium to the eyes repeatedly. Just by applying this collyrium 2-3 times, itching in the swelling reduces.

Lepa – Rasanjana i.e. extract of Berberis aristata and Trikatu i.e. three pungents – long pepper, black pepper and ginger should be rubbed and paste prepared. This paste should be applied to the anjananamika swelling. It reduces itching and suppuration.
